The SHC is OPEN for Sports Physicals/ Well Child exams!


Call anytime between 7:30 a.m. and 4:00 p.m. at (269) 965-9539 to make an appointment.

Services at the SHC

We Will Provide:

First aid/sick care ie: sore throats,earaches,cuts, ect

Lab testing ie: strep throat, cholesterol screening, glucose, ect

Well Child/Preventative care-SPORTS physicals


Monitor chronic illnesses

Referral care for vision/dental/hearing/other services as necessary

Health education

Individual mental health counseling and referrals to other appropriate mental health providers as needed

Confidential Services: counseling, sti and pregnancy testing

Assistance in Medicaid Enrollment


Current Michigan Law allows for confidential (no parental consent) services to minors in these areas:

STI and pregnancy testing


Services NOT provided:

Birth control pills or devices can not be prescribed or dispensed

Abortion counseling or referrals 

Prenatal care



MHSAA Sports Physical

If you (or your child) plans on playing sports and you are making an appointment to have the physical done at the health center please complete and print the MHSAA Sports/Physical Exam AND The Consent for Services forms and bring them to the physical appointment
